Behavioral

At Katy Trail Community Health, we are focused on taking care of the whole person. We recognize our overall wellness is impacted by our physical and behavioral health, which can include mental health, lifestyle behaviors, substance use, addiction, stressors and crisis situations. Services are for children, adolescents, and adults.

Our behavioral health team is located within our primary care settings and is available for our medical patients through virtual and in person visits. We offer a variety of services focusing on prevention, diagnosis, treatment, and recovery. The team includes Integrated Behavioral Health Care Managers whose role focuses on our prevention services, Behavioral Health Providers, Psychiatric Providers whose role focuses on diagnosis, treatment and recovery services, and Behavioral Health Consultant, dedicated to our Primary Care Health Home program.

Prevention:

Screen for and detecting mental illness, substance use, and safety risk for all patients we serve and provide consistent responses to identified need.
Immediate access to behavioral health during medical appointments to provide support, education, assess safety needs and can create with you a personalized plan to meet your care needs. This can include services within KTCH or with our community partners.
Follow up and support through Caring Contacts and Case Management using education and skill building
Enroll in BH services, through an Intake session. This visit collects necessary history and identifies the BH provider that will best meet your needs.

Diagnosis, Treatment, and Recovery.

Complete the diagnostic assessment using information gathered at intake and additional symptom and behavior information to diagnose mental health illness.
Provide Counseling / out-patient individual therapy to identify and address symptoms and behaviors affecting daily living. Assist with developing skills for mood management, coping, emotion regulation, relationships, and behavior modification.
Provide and monitor psychiatric medications to assist with symptom management.
Provide medication-assisted treatment for substance use disorders.
